Translating a Chinese birth certificate (出生医学证明)

The Medical Certificate of Birth is the primary relationship document in most Chinese immigration files. It is short, which makes people careless with it, and it is the parents' block rather than the child's that the reader is checking.

The 出生医学证明 — Medical Certificate of Birth — is issued by the hospital where the birth took place, on a national form administered by the health authority. It has been in national use since 1996, with several form revisions since; the field set below is stable across them, though the layout moves.

It is what USCIS, consulates and universities use to establish parentage. That is worth keeping in mind while translating it: the child's name and date of birth are the part everyone looks at, and the parents' identity block is the part that does the evidentiary work.

Later revisions of the form also carry a barcode and a 打印序列号 (print serial number) distinct from the certificate number. If both are printed, both are transcribed.

The values that must be exact

Things that go wrong

The parents' ID numbers are skipped as "not relevant". They are the most relevant thing on the page. The certificate proves parentage by tying two identified adults to one child, and the identification is the ID number.

民族 rendered as "nationality" next to 国籍 also rendered as "nationality". The document then contains two nationality fields with different values. 民族 is ethnicity; 国籍 is nationality.

出生时间 reduced to a date. The hour and minute are printed on the form. Dropping them makes the translation incomplete, and completeness is the actual legal requirement.

A handwritten attendant's name transcribed as a guess. If it is a signature, write [Signature]. Inventing a name that a verifier cannot match is worse than admitting it is a signature.

The seal skipped because "it's just a stamp". The seal names the issuing hospital. It is the source of the document's authority.

A replacement certificate translated as if it were the original. If the certificate is a 补发 (reissued) copy, the form says so, often with the original number referenced. Say so.

Second and third children

Certificates issued during the one-child policy era occasionally carry annotations relating to birth permits. If anything is written or stamped on the certificate, it goes in the translation. Deciding that an annotation is politically awkward and omitting it converts a complete translation into an incomplete one, and the omission is more conspicuous than the annotation.

Notarial route

For use abroad, the common route is not to translate the certificate loose but to obtain a 出生公证书 — a notarial certificate of birth — from a Chinese notary office, which packages the notary's attestation, a copy of the certificate and, if you request it, an English translation. FAQ

Is the 出生医学证明 the same as a birth certificate?

It is the Chinese equivalent and it is what receiving institutions mean when they ask for a Chinese birth certificate. Translating the title as "Medical Certificate of Birth" and noting that it is the birth certificate issued in China avoids a reader wondering whether they have the right document.

My certificate has no name for the child. Is that a problem?

Older certificates were sometimes issued before the child was named. Translate what is printed — blank stays blank. The hukou then carries the name, which is one reason both documents are usually submitted together.

What if the certificate is lost?

Reissue is handled by the health authority in the province of birth, and for many purposes a notarial certificate of birth based on the hukou and archival records is accepted instead. Which one the receiving institution wants is worth asking before spending on either.

Do I need to translate the back of the certificate?

If anything is printed or written on it, yes. Some revisions carry instructions or a change record on the reverse.

Does the translation need the hospital seal reproduced?
